CANCER PREVENTION AND CARE COORDINATOR

SUMMARY

Under the direction of the Director of Clinic Operations, coordinates all activities of the Cancer Prevention and Care Program (CPCP), including patient navigation, staff training, administration, compliance, outreach related to the program and shortening the time from cancer diagnosis to treatment initiation.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Coordinates CPCP activities at all SBNC medical and dental sites and other sites as determined by community need; trains staff on different cancer prevention program requirements; maintains follow-up records for patients enrolled in the programs
  2. Ensures program compliance with funding contracts, SBNC policies and procedures, and legal requirements; ensures that the appropriate forms are completed for all Every Woman Counts (EWC) visits and are submitted by an appropriate method to the State of California or other data collection site; compiles statistics for required monthly reports; represents SBNC at meetings related to the program.
  3. Performs outreach activities to individuals, provider agencies, and community groups; maintains contact with community agencies that may be interested in our Cancer Prevention and Care Program; collaborates with the Development Department to publicize the services.
  4. Enrolls eligible patients in the Breast & Cervical Cancer Treatment Program and other Cancer Prevention and Care programs. 
  5. Obtain and maintain Covered CA/ Medi-Cal enrollment certification. 
  6. Informs and educates patients and community members of the CPCP at SBNC.
  7. Works with CPCP patients to get proper health coverage.
  8. Arranges Primary Care Provider (PCP) visits for patients referred to the Cancer Center due for any vaccinations such as influenza or other preventative care.
  9. Arranges required dental visit for CPCP patients.
  10. Helps patient with other logistics involved in CPCP including transportation, interpretation, etc.
  11. Helps patient with social service referrals if needed- Food Bank, etc.
  12. Will track CPCP patient follow up calls (performed as needed) to check if they need any resources.
  13. Identify any gaps in care or treatment for the CPCP.
  14. Produces monthly and annual reports on patients enrolled in CPCP and outcome, including days to treatment, dental care, regular PCP care, preventive care (Influenza vaccine, etc), insurance coverage, and patient clinical outcomes. Also collects qualitative feedback from patient about SBNC services.
  15. Performs other reasonably related duties as assigned.

Education:

High school diploma or equivalent.

Experience:

Experience in different databases, follow up tracking, community outreach, health education/advocacy, or an equivalent combination of education and experience.

Knowledge of:

  • Medical terminology.
  • Microsoft Office suite.
  • Community Resources

Ability to:

  • Follow verbal and written instructions.
  • Communicate effectively.
  • Follow up on outstanding items.
  • Learn medical terms related specifically to cancer.
  • Learn how to use NextGen electronic health records system i2i Tracks case management system.
  • Respond and interact with empathy and compassion toward patients experiencing cancer, and their families.
  • Explain medical information in an understandable way.
  • Speak, read, and write Spanish and English.
  • Use basic office equipment including computer, copy machine, fax machine, and postage meter.

Physical Demands:

May be required to lift objects weighing up to 25 pounds. Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard.   Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range. Ability to move about.

Certification/License:

Valid California driver’s license.

Preferred qualifications:

Experience working in a healthcare setting.

QUALIFICATIONS

Current CA RN License in good standing
Current AHA CPR Certification
Bilingual in English/Spanish

Education: Associate or BA/BS Degree

Experience: Five (plus) years of direct patient case experience; community health and/or triage
experience

 Salary Range:  $19 - $23.00 / hour
