SUMMARY

The ECM Medical Social Worker will provide support and guidance to patients and their families in need of emotional, financial, and social assistance. This role involves assessing patients' needs, developing care plans, and connecting them with resources and services to promote overall well-being.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Conduct assessments to determine patients' social and emotional needs
• Develop care plans in collaboration with patients, families, and healthcare providers
• Provide counseling and support to patients and their families
• Coordinate referrals to community resources and services
• Advocate for patients' rights and access to appropriate care
• Maintain accurate and up-to-date documentation of patient interactions
• Participate in case conferences and team meetings to ensure comprehensive care

QUALITIFICATIONS AND EDUCATION REQUIREMENTS

Education: Bachelor's or Master's degree in Social Work. Bilingual in Spanish Preferred.

License: Active state licensure as a Social Worker

Experience: Proven experience working in a medical setting, preferably with ECM patient. Strong communication and interpersonal skills. Knowledge of community resources and support services. Ability to work collaboratively with a multidisciplinary team.

Physical Demands: May be required to lift objects weighing up to 25 pounds. Sitting in a normal seated. Position for extended periods of time. Ability to see within normal parameters. Ability to hear within normal parameters.

Salary Range: $75,000 - $80,000
