SUMMARY:
The Licensed Behavioral Health Provider provides individuals and families with the psychosocial support necessary to cope with chronic, acute or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. Due to client base demographics, English/Spanish bilingual fluency is preferred. This position is an integral part of the patient-centered medical home care team and works collaboratively with others to support optimal patient outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Evaluate and assess patients to determine their needs and the support options that can help them
• Refer patient or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
• Provide patients in individual sessions to assist in recovery from behavioral health issues, including mild to moderate behavioral health diagnoses and substance use disorders.
• Collaborating with other care providers to coordinate services through an integrated care approach.
• Utilize consultation data and behavioral health experience to plan and coordinate patient care and rehabilitation, following though to ensure service efficacy.
• Modify treatment plans to comply with changes in clients’ status.
• Monitor, evaluate and record patient progress according to measurable goals described in treatment and care plan.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:
Education: Master’s level degree in social work or related field plus a valid license in social work or counseling; LCSW, LMFT, LPCC and ASW
Experience: One to Two Years experience in clinical social work/counseling
Knowledge of: Human behavior and performance, individual differences in ability, personality and interests, learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders. Methods, principles and procedures for diagnosis, treatment and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
Ability to: Fully engage with people, taking time to understand their needs through well-established interview techniques. Identify complex problems and review related information to develop and evaluate options and implement solutions.
Physical Requirements: Sitting for an hour at a time with limited breaks between clients. Using computer keyboard or touch screen to enter patient information into EMR system.

SALARY RANGE: $40 - $50
